Jeonju City is achieving success in resolving blind spots in care through an integrated support system that combines medical care and care services.
On the 16th, the city announced that Kim Kyung-hoon, an integrated care case manager, received the '2025 Contribution Award in the Field of Integrated Case Management Development' from the Korean Integrated Case Management Society. Kim, the case manager, was recognized for his efforts in identifying and coordinating high-risk households requiring medical intervention, as well as strengthening data-based case management.
Since 2023, Jeonju City has been implementing a pilot project for integrated medical and care support, establishing various tailored support models such as post-discharge patient health management, home nursing, rehabilitation, and traditional Korean medicine services.
Kim Kyung-hoon, the case manager, expressed his joy in contributing to the establishment of a comprehensive medical and care system, while Lee Geum-an, the head of the elderly welfare department, stated that the dedication of the 15 case managers is the foundation for the development of care services in Jeonju City.
