전북특별자치도의회가 지역 공공의료 체계 전반에 걸친 구조적 위기 상황을 지적하며, 공공의대 설립과 지방의료원 경영 정상화, 의료제도 개선을 위한 실질적인 대책 마련을 한목소리로 요구하고 나섰다.

10일 열린 제423회 정례회 본회의에서는 이정린(남원1), 전용태(진안) 의원이 각각 5분 자유발언을 통해 지역 간 의료격차 해소 및 의료 인프라 불균형 문제를 제기했으며, 염영선(정읍2) 의원은 지방의료원의 경영위기 해소를 위한 정부 차원의 대책 마련을 촉구하는 건의안을 발의해 눈길을 끌었다.

이정린 의원은 “의료 인력이 수도권과 대도시에 편중된 결과, 지역 간 치료 가능 사망률의 격차가 심각한 수준에 이르렀다”며, 지역 공공의료를 살리기 위한 현실적 대안으로 공공의대법의 조속한 처리를 촉구했다.

이 의원은 “충북의 치료가능사망률은 인구 10만명당 49.94명으로, 서울(39.55명)보다 10명 이상 높은 수치”라며, “이처럼 지역 간 생명권 격차가 통계로 드러나고 있음에도 제도적 대응은 지체되고 있다”고 지적했다. 이어 “2018년 당정협의로 결정된 남원 공공의대 설립은 여전히 의사단체의 반발로 표류 중이며, 이는 공공의료 인력의 지역 정착을 위한 시스템 구축이 미루어지고 있다는 의미”라고 밝혔다.

전용태 의원은 진안군의료원이 처한 제도적 불평등 문제를 짚으며 도 차원의 실질적 재정 지원과 제도 개선을 강력히 촉구했다. 그는 “전북의 세 지방의료원 중 유일한 군립 의료원인 진안군의료원만이 국고보조 매칭 시 도비를 전혀 지원받지 못하고 있다”며, “같은 도민을 위한 공공병원임에도 행정 주체가 다르다는 이유로 차별을 받는 것은 명백한 구조적 불평등”이라고 강조했다.

진안군의료원은 2015년 개원 이후 동부산악권의 유일한 공공병원으로 의료 사각지대를 담당해왔으며, 응급의료기관 A등급, 건강검진 최우수기관 등 각종 평가에서 우수한 성과를 거두고 있다. 전 의원은 “이런 병원이 법과 제도의 벽 앞에서 존속 위기를 겪고 있는 것은 도민 생명권에 대한 중대한 위협”이라며, 김관영 도지사의 ‘진안군의료원 재정지원 법제화’ 공약 이행을 요구했다.

염영선 의원은 이날 본회의에서 지방의료원 경영 정상화를 촉구하는 건의안을 발의했다. 그는 “2023년 이후 전국 35개 지방의료원에서 연속된 적자가 누적되고 있으며, 그 원인은 코로나19 감염병 전담병원 지정에 따른 일반진료 중단과 불충분한 국가 보상, 의정갈등으로 인한 의료인력 유출 등 복합적 요인에 있다”고 진단했다.

염 의원은 “지방의료원은 단순한 의료기관이 아니라 지역 보건의료 체계의 핵심 기반인데, 현재 그 기반이 무너지고 있다”며 “정부는 의료진 충원과 운영비 지원 등 현실적인 대책 마련에 즉각 나서야 한다”고 주장했다.

이처럼 세 의원의 발언과 건의안은 각각 다른 지역 현안을 다루고 있지만, 그 핵심에는 공공의료 체계 전반의 불균형·불안정·불평등이라는 공통된 위기의식이 흐르고 있다. 전북도의 공공의료는 인력, 재정, 제도 면에서 삼중의 압박을 받고 있는 가운데, 이를 통합적으로 다루는 전북형 공공의료 종합대책 마련의 필요성이 더욱 절실해지고 있다.

The Jeonbuk Special Self-Governing Province Council pointed out a structural crisis in the overall public healthcare system in the region and called for concrete measures to establish a public medical school, normalize the management of local medical institutions, and improve the medical system.

During the 423rd regular session held on the 10th, Assembly members Lee Jeong-rin (Namwon 1) and Jeon Yong-tae (Jinan) raised concerns about the disparity in medical care between regions and the imbalance in medical infrastructure through their five-minute speeches. Assembly member Yeom Young-seon (Jeongeup 2) drew attention by proposing a resolution urging the government to come up with measures to resolve the management crisis of local medical institutions.

Assembly member Lee Jeong-rin emphasized, "Due to the concentration of medical personnel in the capital region and major cities, the gap in treatable mortality rates between regions has reached a serious level," urging for the prompt handling of the Public Medical School Act as a realistic alternative to save regional public healthcare.

He stated, "The treatable mortality rate in Chungbuk is 49.94 per 100,000 population, which is more than 10 higher than Seoul (39.55)," and pointed out, "Despite the statistical evidence of the gap in the right to life between regions, the institutional response is being delayed." He further expressed, "The establishment of the Namwon public medical school decided through government consultations in 2018 is still adrift due to opposition from medical associations, meaning the establishment of a system for the local settlement of public medical personnel is being postponed."

Assembly member Jeon Yong-tae strongly urged for substantial financial support and institutional improvements at the provincial level while addressing the institutional inequality issues faced by the Jinan County Medical Center, the only public hospital among the three provincial medical institutions in Jeonbuk. He emphasized, "The Jinan County Medical Center, the only municipal hospital among the three provincial medical institutions in Jeonbuk, receives no support from the province when it comes to national subsidy matching," and stated, "Being discriminated against for being a public hospital for the same provincial residents due to different administrative bodies is a clear structural inequality."

The Jinan County Medical Center, the only public hospital in the eastern Busan area since its opening in 2015, has been responsible for medical blind spots and has achieved excellent results in various evaluations such as emergency medical facility grade A and top-tier health checkup facility. The former assembly member stressed, "The fact that such a hospital is facing a survival crisis in front of legal and institutional barriers poses a significant threat to the right to life of provincial residents," demanding the fulfillment of Governor Kim Kwan-young's pledge to legislate financial support for the Jinan County Medical Center.

Assembly member Yeom Young-seon proposed a resolution at the session urging for the normalization of local medical institution management. He diagnosed, "Since 2023, consecutive deficits have been accumulated in 35 local medical institutions nationwide, caused by a combination of factors such as the suspension of general treatments due to the designation of COVID-19 specialized hospitals, inadequate national compensation, and medical personnel outflow due to medical disputes."

Yeom argued, "Local medical institutions are not just simple medical facilities but the core foundation of regional healthcare systems, which is currently crumbling," and insisted, "The government must immediately come up with practical measures such as medical staff recruitment and operational support."

Although the statements and resolutions of the three assembly members address different regional issues, they all share a common awareness of the imbalance, instability, and inequality in the overall public healthcare system. The public healthcare in Jeonbuk is facing triple pressure in terms of personnel, finances, and institutions, highlighting the urgent need for the development of a comprehensive Jeonbuk-style public healthcare plan that deals with these issues in an integrated manner.
